For referring clinicians
How to refer, and what you get back
How to refer
Call the office, or send a note and the relevant records through our referral form. Your patient can also book on their own.
Referral formReason for referral, in a line
Recent notes if you have them
What we send back
A written report after the evaluation, and a short progress note at each progress visit, with your patient's written permission.
Findings, in plain words
The plan, and any change to it
When to expect a visit
Ask the coordinator for the next opening. We do not hold a place in line for a patient who has not called.
Scope
What we do and do not see
We evaluate
How the eyes focus, team, track and shift gaze, alongside a full eye health exam and a glasses prescription.
We train
Those visual skills, in sessions, when the evaluation shows training fits and the patient wants it.
We refer on
Eye disease, anything that needs imaging or medication, and any finding that belongs with a medical doctor. We say so in our report.
We do not treat
The brain, a concussion or any other diagnosis. We do not promise a result, and results vary.
- Sudden vision loss, flashes of light, a curtain over your vision, a chemical splash or an eye injury needs care now. Call our office right away or go to an emergency room. If you can't reach us, call 911.
Send a referral
Call the office or use the referral form. We will confirm with you and your patient.
